The progression of hallux rigidus, a degenerative joint disease affecting the big toe, is typically categorized into four stages. The end stage, also referred to as Stage 4, signifies the most advanced phase of the condition and presents the greatest degree of joint degeneration and symptom severity.
Severe Degeneration of the Joint
In end-stage hallux rigidus, the joint space between the bones is essentially obliterated due to the extensive breakdown of the protective cartilage. This results in almost constant bone-on-bone contact, leading to severe discomfort and significant restriction of movement.
Proliferation of Bone Spurs
At this stage, the formation of bone spurs, or osteophytes, becomes more pronounced and widespread. These bony growths further restrict joint mobility and can give the joint a deformed or enlarged appearance.

End-stage hallux rigidus is characterized by persistent pain, which can occur even during rest. The range of motion in the big toe is drastically reduced, with the toe often becoming rigid. This limited movement can cause significant discomfort, particularly during the ‘push-off’ phase of walking.
